Information for participation in Untire research

Dear Sir/Madam,

Thank you for your interest in the Tired of Cancer research on cancer-related fatigue.

Participation in this research is entirely voluntary. We will be asking you about your personal health situation, among other questions, and therefore will need to obtain your digital consent.

Below are more details about the research and what is expected from you as a participant. Please read this information carefully.

Purpose of this research

The purpose of this research is to get a better understanding of Untire app users, their needs, and interests specific to cancer fatigue. Those insights will be used to help us improve the Untire app.

How is this research set up?

Our research consists of three questionnaires, with a maximum of ten questions for each questionnaire, that will be presented to you throughout a four week period.

What is expected of you?

All participants are asked to fill out the three questionnaires throughout the research. Each will take approximately 5-10 minutes of time.

Upon filling out the first questionnaire, you will be provided with an invitation code for the Untire. With this invite code, you can create an account and start using the app.

What are the benefits of taking part in the research?

We ask that you review the possible benefits and drawbacks of taking part in this research before you decide to join.

Potential Benefits:

• Twelve months free access to the Untire app.
• Using Untire may reduce cancer-related fatigue and increase quality of life.
• Participation in this research will help improve the program for other cancer patients and survivors and this will also help other (former) cancer patients.

A potential drawback could be that filling in the questionnaires takes some time and perhaps energy.

If you do not want to participate or you want to stop participating in the research

Remember, participation is entirely voluntary. If you do decide to participate, you may decide to stop at any time. You can stop participating in this research by sending us an email (support@untire.me) or just not complete the questionnaires. However, if a questionnaire has not been completed within 4 weeks after it has been sent to you, access to Untire will be terminated.

End of the research

Your participation in the research ends when:
• you have completed all the questionnaires
• you decide to stop

Usage and storage of your data

We will collect and use your data: data from the questionnaires that you filled in, data that you entered in Untire, and automatically stored data regarding the use of Untire (log data). Good to know:
- We will only use the data for the PURPOSES mentioned above
- Your data will be processed ANONYMOUSLY and is CONFIDENTIAL
- Your data will be stored on our secured servers
- We wil NEVER sell your data to third parties.
